Notice – Medical Records Announcement

Notice is hereby given that, starting on January 20^th, 2020, Montrose Regional Health will be destroying all x-ray film records prior to 2008, apart from imaging performed on patients who are still minor according to Colorado State Law.  Copies of records must be requested through the Medical Records department prior to Thursday, January 16^th, 2020. A Release of Information Request will need to be submitted. The form can be found on www.MontroseHealth.com website under Patients and submitted to Montrose Regional Health Medical Imaging Department at 800 South Third Street, Montrose, CO. 81401.  Or stop at the Front Desk on Level 1 for a Release of Information Request form.
